Position: Patient Care Coordinator

Weeneebayko Area Health Authority (WAHA) is seeking experience candidate to fill the key role of Patient Care Coordinator
The Organization
Weeneebayko Area Health Authority (WAHA) is the leading health care provider on the western James Bay coast. WAHA comprises of hospitals in Moose Factory, Fort Albany, and Attawapiskat, with a Health Centre in Moosonee. WAHA also provides regional outpatient programs, as well as Paramedicine services in Moose Factory, Moosonee, Fort Albany, Kashechewan, Attawapiskat, and Peawanuck. The span of services, complexity of care needs in the region, and the depth of health integration has made WAHA a leader and a model of health care services in Northern Ontario. WAHA consists of 450 employees, a Physician team, many students, as well as community stakeholders. It is affiliated with Queens University and offers many educational opportunities for current and prospective team members.

Key Responsibilities:
• Program Development/ Improvement Leadership
• Perform client assessments and initiate intervention, using best practices and clinical practice guidelines
• Ensure client confidentiality and privacy is always maintained
• Develop, implement, and maintain decision-support tools specific to the clinic, and work with other programs and/or systems on tools and protocols addressing broader issues outside local primary care clinic.
• Assess client/population needs and develops and/or delivers health education with multiple strategies - focusing on health promotion and client independence.
• Possess knowledge of health systems and program access processes, and maintains effective working relationships with other health service providers Carry out clerical duties
• Health & Safety/Infection Prevention & Control
Qualifications/Experience:
• Diploma in Nursing required, Bachelor of Science in Nursing Preferred
• Registered with College of Nurses of Ontario and in good standing
• Certification in BLS and other mandatory in-services prescribed by Weeneebayko Area Health Authority
• Knowledge of the organizational structure and health system in the region
